NHS THEMES

Tele-HEalth Monitoring following dischargE after Stroke (THEMES) is a project which involves the stroke team at the Countess of Chester NHS Foundation Trust and the community stroke team keeping in regular contact with the patient once they are discharged from hospital.

Currently patients are given an appointment to see the doctor 6 weeks after they are discharged – but have no medical contact until then (unless due to an emergency). With the development of this app, the stroke team can monitor all aspects of rehabilitation and will also observe a patients social and financial well-being.

Please note that this app is only available to patients participating in this research.
