各時段門診請於門診診療時間30分鐘以前至診室候診

如不克前來門診請提前一日取消掛號，看診當日恕不接受取消掛號及換號

初診病人請於看診當日攜帶身份證及健保卡，
請至批價掛號櫃檯填寫「初診基本資料表」，辦理初診報到手續
