Вызов врача

Приложение "Вызов врача" разработано специально для пациентов Медицинского центра г. Якутска (бывшие городские больницы N4 и N5).
Заявки принимаются только по будням с 8:00 до 15:00, в субботу и воскресенье не принимаются.
Теперь вам не нужно дозваниваться до поликлиники, чтобы вызвать врача на дом. Достаточно скачать приложение себе на мобильное устройство и заполнить форму.
Порядок заполнения формы
1. Выбираем специалиста. Если заболел взрослый человек — выбираем "терапевт", если врач нужен ребенку — "педиатр"
2. Вводим название улицы. При вводе первых букв названия улицы появится выпадающий список улиц, из которого нужно выбрать вашу (если вашей улицы в списке нет, значит ваш дом обслуживается другой поликлиникой). Далее вводим номер дома, этаж, подъезд и квартиру.
3. Далее в поле ФИО нужно ввести полностью фамилию, имя и отчество пациента и внизу прокруткой выбрать дату рождения пациента
4. Нажимаем "Далее"
5. Вводим номер телефона
6. В поле "Жалобы" вводим симптомы, которые беспокоят пациента. Например, "температура 38.5, кашель, головная боль".
7. Проверив правильность заполненных данных, нажимаем "Отправить"
8. Появится номер вашей заявки, ее статус и время добавления
9. Через некоторое время вам придет уведомление и статус заявки изменится на "Передано". Это означает, что ваша заявка обработана и поступила к врачу.
10. Ожидаем врача.
Внимание! В некоторые дни ввиду большого количества вызовов время ожидания врача может превышать 2 часа. Если статус вашей заявки — "передано", беспокоиться не стоит, врач в любом случае приедет.
Если пациент не в состоянии ждать врача более двух часов, необходимо вызвать скорую помощь. Appendix "Call the doctor," specially designed for the patients of the Medical Center of Yakutsk (former city hospital N4 and N5).

3. Next, in the Name, enter full name, first name and patronymic of the patient and the bottom of the scroll to choose the date of birth of the patient
4. Click "Next"
5. Enter the phone number
6. In the "Complaints" enter the symptoms that bother the patient. For example, "temperature is 38.5, cough, headache."
7. check the correctness of the data, press "Submit"
8. The number of your application, its status and the addition,
9. After some time, you will be notified and the status of the application changes to "Submitted". This means that your application is processed and received medical attention.
10. Expect the doctor.
Attention! On some days, because of the large number of calls while waiting for the doctor may exceed 2:00. If the status of your application - "transferred", do not worry, the doctor will come in any case.
If the patient is unable to wait for a doctor for more than two hours, you need to call an ambulance.
