Visit Planner

The VisitPlanner is a tool designed to help parents of children with Inflammatory Bowel Disease manage their child’s health and optimize their engagement in the health care process. It encourages parents to observe their child’s health condition and to keep track of important data to discuss with their child’s care providers. The tool is designed to be used routinely throughout the time period between clinic visits. It incorporates ongoing assessment in between visits with focused preparation in the week prior to a scheduled clinic visit. The tool uses notifications to prompt parents to complete brief assessments of their child’s health status once per week, on random days. Parents also receive weekly notifications regarding any non-urgent questions or concerns they would like to remember to address at their child’s next clinic visit. One week prior to a scheduled clinic visit, parents are able to view a summary of the data they have recorded and complete a pre-visit plan that is returned to the app in report form to be used at their visit and also delivered electronically to their healthcare team.
