Healthy Transitions

The Healthy Transitions app helps prepare adolescents and young adults (ages 14-30) with special healthcare needs, for the transition from the pediatric, to the adult health care system. Through video training sessions and short quizzes, users will learn about the transition process, and how to take responsibility for their health.
