Zorg4Zeist app van huisartsen in de regio Zeist.

Zorg4Zeist app is een variant van de Uw Zorg online app voor de regio Zeist. Hiermee heeft u 24 uur per dag toegang tot uw medicatieoverzicht, kunt u eerder voorgeschreven medicijnen bestellen, afspraken maken en een eConsult starten bij uw huisarts! Een overzicht van aangesloten praktijken vindt u in de app. Wij zijn benieuwd naar uw ervaringen om zo de app verder te kunnen optimaliseren. Hoe kan ik de app in gebruik nemen?
1. Download de app uit de store
2. Open de app, doorloop de uitleg en selecteer uw praktijk
3. Druk op de knop 'toestel koppelen'
4. Als u al een account heeft voor een patiëntportaal bij uw huisarts logt u met dezelfde gegevens in op de app door op de knop 'Aanmelden' te drukken en gaat u direct naar stap 5). Als u nog geen account heeft, vraagt u die bij ons aan door op de knop 'Registreren' te klikken en alle benodigde gegevens in te vullen. Na controle van uw aanvraag door onze praktijk -wat enige tijd in beslag kan nemen- wordt uw account aangemaakt en ontvangt u per e-mail een bericht dat u gebruik kunt maken van de services
5. Na inloggen op de app ontvangt u per e-mail of sms éénmalig een verificatiecode die u invoert in de app
6. Maak in de app ten slotte een pincode van 5 cijfers aan om de toegang af te schermen
7. De app is nu klaar voor gebruik

Functionaliteiten
• Inzage in uw actuele medicatieprofiel zoals bekend bij uw huisarts.
• Vraag herhaalrecepten direct uit uw medicatielijst aan en ontvang herinneringen indien u uw medicatie opnieuw nodig heeft.
• Stel uw medische vragen direct aan uw arts via een eConsult en ontvang bericht zodra uw consult beantwoord is. Let op! eConsult is niet bedoeld voor urgente zaken of levensbedreigende situaties. Indien u twijfelt over de ernst van uw klacht, neem dan altijd telefonisch contact met de huisarts op.
• Bekijk de lege plekken in de agenda van uw arts en boek een afspraak op het moment dat het u uitkomt. U moet tevens de reden van uw afspraak vermelden.
• U vindt de adresgegevens, contactgegevens en openingstijden van uw arts in de app. U vindt er ook een link naar de website van uw arts.

Privacy
Met de app kunt u via een beveiligde verbinding uw medicatie gegevens ophalen uit het systeem van de praktijk en communiceren met uw arts. Voor de ingebruikname wordt uw identiteit eerst geverifieerd door de praktijk en ontvangt u een verificatiecode om de app te activeren. Verder dient u de app afschermen door een persoonlijke 5-cijferige pincode. Uw gegevens worden niet gedeeld met derden. Zorg4Zeist app is a variant of the Make Your online app for Zeist region. This will have 24 hour access to your medication list, you can order previously prescribed medications, appointments and start eConsult your doctor! How can I start using the app?
1. Download the app from the store
2. Open the app, go through the explanations and select your practice
3. Press "Pair device" button
4. If you already have an account log for a patient portal to your GP with the same data in the app by pressing the button "Register" and go directly to step 5). If you do not have one, ask to us by clicking on the 'Register' button and fill out all required information. After reviewing your application created by our practice -what some may nemen- up your account and you will receive an email that you can use the services
5. After logging into the app you will receive an email or SMS one-time verification code that you enter in the app
6. In the app finally a 5 digit PIN to foreclose access
7. The app is now ready for use
 
functionalities
• Access to your current medication profile known as your family doctor.
• Ask for prescription refills directly from your medication list to receive reminders if you again need your medication.
• Ask your medical questions your doctor immediately via eConsult and get your message once consultation has been answered. Pay attention! eConsult is not intended for urgent matters or life-threatening situations. If in doubt about the seriousness of your complaint, you should always call the GP.
• Check the blanks in the calendar of your doctor and book an appointment at the time that suits you. You must also state the reason for your appointment.
• You will find the addresses, contact details and opening hours of your doctor in the app. You will also find a link to the website of your doctor.
 
Privacy
With the app you can use a secure connection to retrieve your medication data from the system of practice and communicate with your doctor. Before commissioning your identity is verified by practice and you will receive a verification code to activate the app. You should also shield the app through a personal five-digit PIN.
