Dit kan onder andere door middel van eerstelijnsverblijf (ELV), maar er zijn ook diverse andere zorgvormen mogelijk. Het triage-instrument in de app “ZorgbedMB” ondersteunt u bij het maken van de juiste keuze van aanvullende zorg. Door middel van het doorlopen van het triage-instrument ziet u welk zorgvorm het best aansluit bij de zorgvraag van uw patiënt. Indien het eerstelijnsbed de meest geschikte zorgvorm is, toont de app een actueel overzicht van de op dat moment beschikbare ELV-bedden in Midden-Brabant.
Naast het triage-instrument en het overzicht van de beschikbare ELV-bedden biedt de app ook een consultatiefunctie aan. Het geeft u de mogelijkheid contact op te nemen met een specialist ouderengeneeskunde van een van de VVT-organisaties in Midden Brabant en/of de geriater van het Elisabeth-TweeSteden Ziekenhuis.
