Tropaz LabWest

Patiënten die bij LabWest afdeling Trombosedienst onder behandeling zijn, kunnen na autorisatie, gebruik maken van de Tropaz app. Hierbij kunnen de meest actuele gegevens uit het dossier worden opgevraagd zoals de INR waarde, de dosering en de afspraakdatum voor INR-controle. Voor zelfmeet-patiënten is het met de Tropaz app ook mogelijk een INR waarde door te geven. Voor autorisatie kunt u contact opnemen met LabWest afdeling Trombosedienst.

WAARSCHUWING:
LabWest geeft u toegang tot uw doseerkalender welke deel uitmaakt van uw patiëntendossier. Door uw verzoek tot toegang via de app wordt u geacht uitdrukkelijk in te stemmen met de verstrekking door LabWest van uw persoonsgegevens uit uw patiëntendossier.

LabWest waarschuwt voor het eventuele risico dat uw persoonsgegevens via de app in handen komen en blijven van derden. LabWest aanvaardt hiervoor geen enkele verantwoordelijkheid en gaat er vanuit dat u zelf zorgdraagt voor de bescherming van uw persoonsgegevens vanaf het moment dat deze via de app vanuit het netwerk van LabWest worden ontsloten. LabWest draagt evenmin verantwoordelijkheid voor fouten in uw persoonsgegevens of vertraging in de verzending daarvan, vanaf het moment dat deze via de app het netwerk van LabWest hebben verlaten.

LabWest waarschuwt ook voor eventueel onbevoegd gebruik van uw mobiele telefoon of ander apparaat waarmee u de app kunt activeren. Als gevolg daarvan kunnen uw persoonsgegevens in handen van een onbevoegde komen. Dat kan ook het geval zijn, indien u de app niet afsluit en uw mobiele telefoon of ander apparaat waarmee u de app kunt activeren, wordt gestolen of verloren. Of indien uw telefoon of ander apparaat waarmee u de app kunt activeren, wordt gestolen of verloren, terwijl uw persoonsgegevens daarop zijn achtergebleven.

U krijgt toegang tot gegevens zoals uw doseerkalender door op installeren te klikken, waarmee u verklaart kennis te hebben genomen van deze waarschuwing. Patients being treated with LabWest department Thrombosis may after authorization, use the Tropaz app. Here, the most current data may be retrieved from the file, such as the INR value, the dose and the appointment date for INR control. For self-measurement with the Tropaz patients it is app also possible to pass an INR value. For authorization, please contact LabWest department Thrombosis.
