栃木県宇都宮市の身体障害者療護施設『ハートフィールド』のアプリです。
社会福祉法人藹藹会は、平成１２年４月 身体障害者療護施設ハートフィールドを開所し、
主に障がいのある方への支援に取り組んできました。
平成１７年に自立支援法が制定され３障がい一元化により、障がいをもった様々な方々の
支援をおこなってきました。平成２５年４月総合福祉法制定に伴い、
指定特定相談支援「ポンテ」をオープンしました。
今後も、相談支援事業と従来のサービスの特性を活かし、宇都宮市及び近隣の市町の障が
いをお持ちの方、ご家族のライフストーリーの一端を担えるよう努力していきます。
