（旧:電子お薬手帳システム for finemedical）
薬局様、医療機関（院内処方）様、歯科様等から
提供されるお薬手帳を一元管理できますし、薬局
様への処方箋画像の送信もでき、かつ、患者様の
健康維持や改善に役立つ他の機能もあります。
患者様は無料でご使用できます。
また、パソコンでも同様な機能が無料でご使用で
きます。
パソコン版をご使用したい場合は、メールにて、

今回の機能を使用すると以下のメリットを受けら
れます。
＜メリット＞
①紙手帳の代わりにスマートフォンでお薬手帳を
一元管理できます。
　薬局様、医療機関（院内処方）様、歯科様等か
　らＱＲコードでお薬手帳のデータを頂いた場合
　はＱＲの読み込みをして、登録すると弊社のク
　ラウドサーバに送信され、データをお預かり致
　します。
　また、ＱＲではなく、紙で頂いた場合は、その
紙手帳を、スマートフォンのカメラ機能で撮影
し、登録すると弊社のクラウドサーバに撮影画
像が送信され、データをお預かり致します。
　尚、ＱＲコードはＪＡＨＩＳ電子版お薬手帳デ
　ータフォーマット仕様書Ｖ１．０　、１．１、
　２．０に対応しております。
②ＭＡＸ９人分の登録ができますので、家族全員
　分が１つのスマートフォンで管理できます。
③お預かりしたデータは弊社のクラウドサーバで
　管理致しますので、データの制限を気にする必
　要はありません。
　また、スマートフォンが故障しても、又は機種
　変更しても、データは継続できる仕組みを持っ
　ています。
④お薬手帳の表示で、前回との比較を自動で行い、
　その結果をマーク（■、▲、▼）表示しますの
　で、前回に比べ薬が増えた及び薬の量が減った
　等の相違が判断できます。
⑤食事の時間を設定すると、服用時点に通知機能
とアラームでお知らせしますので、飲み忘れの
防止に役立ちます。
⑥検査値は人間ドックの検査項目をほぼ網羅する
約４０項目を用意していますので、即使用でき
ます。
　また、検査結果を入力することで、基準値を含
めてグラフ表示されますので、健康の推移が一
目で判ります。
⑦症状から病名を検索したり、近隣の病院を探し
たり等のお役立ち情報として約１５項目が利用
できます。
⑧処方箋を撮影し、その画像を薬局様へ送信がで
　きます。

＜注意事項＞
①個人情報の登録は保険証の内容と同じ内容で登
　録して下さい。
　そうしないと、お薬手帳との紐づけが正しくで
　きない可能性があります。
②お薬手帳はＱＲデータ及び紙のお薬手帳ともに
　、登録した時点で弊社のクラウドサーバに登録
　されますので、登録漏れや、通信エラー等で、
　データの登録漏れが無いように注意をお願いし
　ます。
③本アプリの使用にあたっては、プライバシーポ
　リシー及び利用規約を必ず、一読し、同意して
　からご使用をお願い致します。
④検査値の基準値は統一されておらず、発表する
医療機関等で多少相違がありますので、あくま
でも目安として下さい。
⑤ドメイン名@health-care-fm.jpからのメール受
　信が可能な設定にして下さい。
⑥処方箋画像の送信は薬局様の設定で、時間外は
　送信できない可能性があります。
⑦処方箋画像の送信で、10分以上経っても、受付
　完了メールが届かいない場合は、⑤の設定の問
　題か、または、通信の問題等で、薬局様に届い
　ていない場合がりますので、薬局様に問い合わ
　せしてください。
