Huddle for Care

Huddle for Cares is a web-based platform that fosters online sharing and learning between professionals working to strengthen care transition. Our goal is to accelerate the spread of ideas across the country by creating a forum in which transitional care implementers can share their stories and tools they found effective in overcoming challenges and meeting goals. Huddle for care is a community within and beyond local areas that fosters mutual support and collaboration across programs in different hospitals.
