Met de App Risicoscan kunnen zorgmedewerkers in de ouderenzorg eenvoudig en snel gezondheidsrisico’s bij ouderen signaleren. De app geeft suggesties voor vervolgacties en praktische adviezen om de zorg te verbeteren.

Deze app voor risicosignalering, ontwikkeld door het Kennisplein Zorg voor Beter, is voor zorgmedewerkers in verpleeg- en verzorgingshuizen, en voor thuiszorgmedewerkers.

Is er een verhoogd risico?

Met de app beantwoord je vijf vragen voor de gezondheidsrisico’s: ondervoeding, medicatiefouten, mondproblemen, huidletsel, incontinentie, depressie en vallen. Daarna geeft de app aan of er een verhoogd risico bestaat, en of er mogelijk ook andere gezondheidsrisico’s zijn, die je vervolgens meteen kunt checken. Bijvoorbeeld: een risico op ondervoeding, kan ook sneller leiden tot huidletsel of vallen.

Print het resultaat

De resultaten van de Risicoscan kan de zorgmedewerker vanuit de app als pdf aan zichzelf mailen, printen en toevoegen aan het dossier of zorgleefplan van de cliënt.
