adults to move from home to assisted living or nursing home. Take the quiz to find out if you have any of
the 7 risk factors. By knowing your risks you can be proactive and address them before they could cause
you to move. Or take the quiz just to learn what the risks are. This quiz is the first step in the 4-step Live
Well at Home process that helps older adults “find your way” to live well at home.
