Developed in partnership with the Heart Failure Society of America, this app makes it easy to record your symptoms, vital signs, medications, and more to help you manage heart failure and congestive heart failure (CHF). Choose what you want to track to build your own summary “My Storylines” to learn more about your health, and to share more – safely and securely – with your doctor about what happened between visits.

I generally find this app very useful but it keeps shutting down. It works fine sometimes but can't get in to it at all at other times. It's frustrating.

Overall pleased with app. Very good app. But until there is an edit/delete button for appointments section (I don't see any way to delete/edit entries), I can only give 4 stars. Add this and I can give it 5 stars. Good job with this app

Spyware. Description should make it clearer that this is a front end for collecting dosiers on people. Should give option to use functions anonymously, without sending information to their database. It is really slow to start, got as far as creating profile, and just sat there.
