要介護度チェッカー

介護保険制度の要介護認定における一次判定をお手持ちのAndroid端末で行えるアプリケーションです。

要介護度認定は、訪問調査結果および主治医意見書による一次判定が行われ、その一次判定結果を参考に介護認定審査会による二次判定により決定されます。
要介護度チェッカーでは、訪問調査による一次判定結果の目安を知ることができます。
また、定期的に調査を行うことにより被介護者の状態の推移を把握することができます。

※平成24年2月現在最新の要介護度判定方法（厚生労働省の資料「介護認定審査会委員テキスト2009改訂版」）に基づいて要介護度を算出します。

※主治医意見書を入力しなくとも基本的な要介護度が算出できます。主治医意見書が未入力の場合は認知症加算が反映されない状態となります。


主な機能
・最新の方法に沿った要介護度チェック（一次判定）が行えます。
・調査内容および判定結果はデータとして端末に保存されます。
・介護認定審査会資料とほぼ同じ判定結果画面が出力されます。
・複数人分の情報を管理できるため介護事業所でも有効活用できます。

※ I can calculated the level of care required basic without having to enter the doctor opinion. It is a state that is not reflected dementia add an additional doctor written opinion is not entered.


Main function
· I can make (primary decision) Check the level of care required in line with the current methods.
And judgment and results survey content will be stored in the terminal as the data.
And judgment result screen similar to the committee for certification of need document is output.
• You can effectively use in the nursing office because you can manage the information of multiple persons.
