HCP Connect

HCP Connect is an intuitive, interactive, and customisable application that was designed to manage and provide supportive care for chronic patients at home. This application is restricted to credentialed care providers who are managing patients experiencing various chronic diseases including, among others, cancer and CHF. The application encompasses many innovative and tightly integrated tools - all together, all in one place. Additional tools instrumental to supportive patient care will be available in future updates.

The platform provides the following features:
