البهاق




تعريف






المسببات



وهناك عدة نظريات من المحتمل أن تكون هي السبب في الإصابة بالبهاق وهي:

1. حدوث خلل في وظيفة الخلايا الصبغية نتيجة لخلل في الأعصاب المغذية لها.

2. تفاعل مناعي ذاتي يؤدي إلى تعرف الجسم على الخلايا الصبغية على أنها خلايا غريبة عن الجسم فيتعامل معها ويدمرها

3. تدمير الخلايا الصبغية لنفسها

ويكتشف مريض البهاق الإصابة بنفسه من خلال تباين اللون بين الأماكن المصابة والأماكن السليمة. ولا يمكن للمريض أو الطبيب تحديد مدى الإصابة أو كمية الخلايا الصبغية التي ستفقد أثناء الإصابة بالمرض


الأعراض




إن مرض البهاق غالباً ما يصيب الأفراد الذين لديهم تاريخ وراثي لهذا المرض ( أي إصابة أحد أفراد العائلة ) وغالباً ما تبدأ الإصابة قبل بلوغ سن العشرين

العلاج

هناك عدة طرق علاجية لمرض البهاق تتلخص في

1. الوسائل الموضعية: مثل الدهانات أو الحقن الموضعي للجلد

2. العلاج بالأشعة الفوق بنفسجية

3. العلاج الجراحي: مثل زراعة الخلايا الصبغية أو تطعيم الأماكن المصابة بجلد سليم. وتستخدم هذه الطريقة في البهاق الثابت والغير مستجيب للعلاج بالطرق الأخرى


5. استخدام الدهانات الواقية من أشعة الشمس في معظم حالات البهاق خوفاً من حدوث الحروق الشمسية



أنواع البهاق


1. البهاق المنتشر

وهو الذي يظهر وينتشر تدريجياً ليصيب مساحات كبيرة من الجسم قد تصل إلى كامل الجسم ماعدا أجزاء بسيطة تحتفظ بلونها الأصلي


2. البهاق الثابت أو المستقر

وهو الذي يبدأ ثم ينتشر في أجزاء معينة ثم يتوقف عن الانتشار بحيث لا تزيد المساحات المصابة بعد التوقف


3. البهاق المتراجع

وهو الذي يبدأ وينتشر ثم يتراجع تدريجياً وتبدأ الصبغة في الظهور مرة أحرى في الأماكن التي أصيبت بالبهاق

السكتة الدماغية




تعريف


هي عدم وصول الدم بشكل مفاجئ الى المخ، وهي حالة طبية طارئة قد ينتج عنها شلل.

المسببات


تحدث بسبب توقف الدورة الدموية في الدماغ، ويحدث هذا التوقف اما لحدوث خثار دماغي أو انسداد دماغي. ويحدث هذا عند حدوث جلطة دموية في احد الشرايين الرئيسية التي تنقل الدم الى المخ. أو نتيجة اصابة الانسان بمرض السكري، أو ارتفاع نسبة الكوليسترول في الدم الذي ينتج عنه تصلب الشرايين. ومن الأسباب الرئيسية الأخرى لهذا المرض هو النزف الدماغي، ويحدث نتيجة انفجار شريان دموي في المخ.




وسائل العلاج


أهم من العلاج هو الوقاية من المرض، وأهم سبل الوقاية لهذا المرض هو فحص ضغط الدم باستمرار للمرضى المصابون بضغط الدم وأخذ الدواء المناسب. ويعتمد العلاج على منع المضاعفات وتبعاً للعجز الذي يصيب المريض. واعادة التأهيل بعد الاصابة ويتوقف نجاح اعادة التأهيل على مدى قدرة المريض على التعاون مع أطبائه. ويتلقى مرضى السكتة الدماغية علاج أساسي من ثلاثة أنواع من الأطباء :


1- أخصائي العلاج الطبيعي: حيث يساعد المريض المصاب بالشلل على تحريك أطرافهم المصابة لمنع تصلب العضلات وذلك بالتمارين الرياضية والحرارة والتدليك.

2- أخصائي علاج عيوب النطق: حيث يساعد المرضى الذين فقدوا النطق بسبب السكتة الدماغية.
