About Providence E-Card

Thank you for downloading Providence Health Plan’s mobile member ID card.

With this app you will be able to:
· View your most recent ID card
· View the ID cards for your spouse and or dependents on your policy
· Email or fax any ID card on your policy to a doctor office, hospital or pharmacy

To get access:
· Download the app
· Enter your member ID number
· Enter your 4-digit PIN. For ID cards printed after June 20, 2011, the PIN is located on the back of your ID card (lower right corner). For cards printed prior to June 20, 2011, members may request PIN from Providence Health Plan Customer Service at 503-574-7500 or 800-878-4445 or by secure email form at www.providence.org/php/contactus (you must provide member ID number for authentication).

Got ID and pin from providence, entered and saved, works like a charm. It's sole purpose is to show you your card, and that is exactly what it does. Not sure why so many people can't figure it out! It's not rocket science. Call providence and get the ID and pin if you can't figure it out.

Firstly, the PIN is on the lower right back of your card. Second, save your login information. Boom. Now you can leave your card at home or in your second wallet. It crashed after I selected my name after login. However, when I tried the second time I had no problems. If they can fix the crashing issue then they are golden.

App will not work either on our android or iPhone. It keeps stopping before generating an I'd card. Tried installing 4 different times

PIN # is on BACK SIDE of Card in lower right corner. On my card, the word "benefits" overlaps this 4-digit small font PIN #. Very stupid to overlap PIN # with text and make it hard to read. Saving login info saves both the ID # and the PIN #.

USELESS. There is no point for this app to exist. This requires me to have my card at all times or remember the ID# and the PIN#. Then why would I need this app? It does not even remember this information. Have to enter it every-time. Okay it can fax/email to someone. If already have a card, I can do this myself using my phone's camera. Other institutions are providing a way of carrying an e-card that is easy to access -- no remembering ID/PIN# s or needing a physical card. USELESS. Also, it should have given a hint as to where to find the PIN#. Now I know after googling that the PIN# is on the back of the card. If it is NOT a big secret, why not help out the user a bit. Again USELESS. I would rather carry pictures of my card myself... problem solved, saved space, no need for a internet connection.

Just what I wanted--no longer have to carry physical cards. To find your PIN, look at "Read More" in the description.

Did while standing in line at pharmacy. Call Providence and ask for pin. Takes 30 seconds. I even faxed my doctor and pharmacy my card from my phone. Some common sense or the use of your eyeballs and you would of seen the directions and be able to read them in the APP!!!!

Displays an image of your card(s). Works on Motorola phone. Installs on Samsung Galaxy S tablet but crashes instead of loading the card.
