NOTA 13 (Cuore e Colesterolo)

La NOTA 13 AIFA (Agenzia Italiana del Farmaco) è un testo articolato e complesso, in continuo aggiornamento e quindi non facile da conoscere ed applicare. Questa applicazione è un soccorso al medico prescrittore: in 4 semplici step di inserimento dati si può stratificare il rischio del paziente e identificare la terapia ottimale.
STEP 1: DATI PAZIENTE
Semplicemente valori di colesterolo totale ed LDL, sesso e abitudine tabagica
STEP 2: CONDIZIONI ASSOCIATE
Condizioni che da sole stratificano il rischio come alto o molto alto (una check-list di 8 elementi, raggruppati in 4 categorie).
STEP 3: DISLIPIDEMIE FAMILIARI
Diagnosticate in base a dei criteri semplificati: dapprima sono testati i criteri biochimici (è richiesto, aggiuntivamente, solo il range di trigliceridemia) e, se questi sono compatibili, vengono richiesti anche alcuni criteri anamnestici-familiari (una breve check-list di 2-5 elementi).
STEP 4: CARTE DEL RISCHIO
In caso di negatività di tutti gli step precedenti, il rischio è stratificato in base alle “carte del rischio” ESC, incrociando i valori di colesterolo totale, già inseriti, con quelli della fascia d’età e del valore di pressione arteriosa sistolica
RISULTATO
A questo punto viene fornito il risultato finale:
1. rischio stimato per il paziente (molto alto, alto, moderato, medio, lieve) e mortalità cardiovascolare attesa a 10 anni;
2. target terapeutico LDL, ossia valore ottimale da raggiungere per quel paziente;
3. percentuale riduzione LDL necessario per raggiungere il target;
4. terapia indicata, secondo livelli progressivi, in base alla risposta iniziale; sono indicate anche le eventuali “terapie aggiuntive” per ottimizzare il profilo lipidico (HDL, trigliceridi), oltre al controllo del target di LDL;
5. un tasto “DOSE” apre l’istogramma rappresentativo della potenza delle statine nei vari dosaggi, aiutando nella scelta, a seconda della percentuale di abbattimento LDL necessaria calcolata.

Questa applicazione ricalca IL PIU’ FEDELMENTE POSSIBILE le indicazioni della nota AIFA, tendo conto che, per la complessità dell’argomento, si possono trovare condizioni non chiaramente codificate dalla nota, spesso per mancanza effettiva di evidenze scientifiche relative a problematiche particolari; ad esempio non si trova una chiara indicazione del cutt-off di valori di Trigliceridemia da trattare.
Per approfondimenti si consiglia consultare direttamente il testo della Gazzetta Ufficiale. E’ da rimarcare, inoltre, come la stratificazione del rischio proposta dal Ministero si ispiri, ma non ricalchi completamente quanto suggerito dalle linee guida ESC.
Si sottolinea infine come l'applicazione delle norme della NOTA 13 non sia un mero adempimento burocratico ma un atto medico finalizzato alla stratificazione prognostica del paziente e alla formulazione della terapia più adeguata, con notevoli implicazioni sull'outcome del paziente a lungo termine.

4. indicated therapy, according to progressive levels, based on the initial response, are also shown any "adjunctive therapies" to optimize the lipid profile (HDL, triglycerides), in addition to control of the target LDL;
5. a button "DOSE" opens the histogram represents the power of statins in various strengths, helping in choice, depending on the abatement percentage calculated LDL required.
